Create the Form**\n\n1.  Go to **StoreConnect Navigation** → **Configuration** → **Forms** → **All Forms**.\n\n2.  Click **New** to create a form.\n\n3.  Select **Checkout Form** type → Click **Next**.\n\n4.  Give your form a **Name**.\n\n5.  In **Display Mode**, choose when to show the form:\n\n    -   **During Checkout**\n\n    -   **After Order is Complete**\n\n6.  Click **Save**.\n\n\n---\n\n### **2\\. Add Form Questions**\n\n1.  Go to the **Related** section of the form.\n\n2.  Click **New** to add a question.\n\n3.  Enter the **Question Text**.\n\n4.  Choose **Data Type** (e.g. text, pick list, date, number, file upload).\n\n    -   If using **Pick List**, enter options separated by semicolons `;`.\n\n5.  Set **Position** in the form.\n\n6.  Optionally mark **Required** if the customer must answer it.\n\n7.  Click **Save and New** to add more questions.\n\n\n---\n\n### **3\\. Apply the Form to a Store**\n\n1.  Go to **Store Forms**.\n\n2.  Click **New**.\n\n3.  Select your **Store**.\n\n4.  Set the form to **Active**.\n\n5.  Click **Save**.\n\n\n---\n\n### **4\\. Control Form Display (Optional)**\n\n-   Use the **Conditions** field to control where the form appears in checkout:\n\n    -   Example conditions:\n\n        -   **Customer Information**\n\n        -   **Shipping Information**\n\n        -   **Terms and Conditions**\n\n        -   **Payment Information**\n\n\n---\n\n### **5\\. View and Test the Form**\n\n1.  Go to your **Storefront Checkout** and refresh the page.\n\n2.  Check that the form appears in the selected checkout step.\n\n3.  After checkout, if set to display **after checkout**, the form will appear on the **Order Page**.\n\n\n---\n\n### **6\\. View Form Responses (Backend)**\n\n1.  Go to **Orders** in backend.\n\n2.  Open the desired **Order**.\n\n3.  Go to **Order Products** → select product → scroll down.\n\n4.  See **Form Submissions** and view responses."}
