Create the Form**\n\n1.  Go to **StoreConnect Navigation** → **Configuration** → **Forms** → **All Forms**.\n\n2.  Click **New** to create a form.\n\n3.  Choose **Product Form** type → Click **Next**.\n\n4.  Give the form a **Name**.\n\n5.  Click **Save**.\n\n\n---\n\n### **2\\. Add Form Questions**\n\n1.  Go to the **Related** tab of your form.\n\n2.  Under **Form Questions**, click **New**.\n\n3.  For each question:\n\n    -   Enter the **Question Text**.\n\n    -   Select the **Data Type**:\n\n        -   **Boolean** (Yes/No)\n\n        -   **Number**\n\n        -   **Text** / **Text Area**\n\n        -   **Date**\n\n        -   **Pick List** (options separated by `;`)\n\n        -   **File Upload**\n\n    -   Optionally check **Required** to make the field mandatory.\n\n    -   Set **Position** to control the order of the list.\n\n4.  Click **Save and New** to add more questions as needed.\n\n5.  Repeat until all questions are added.\n\n\n---\n\n### **3\\. Associate the Form with the Product**\n\n1.  Go to **Product Forms**.\n\n2.  Click **New**.\n\n3.  Select the **Product** (e.g. *Steve Plushy*).\n\n4.  Click **Save**.\n\n\n---\n\n### **4\\. Associate the Form with the Store**\n\n1.  Still in **Product Forms**, select your **Store**.\n\n2.  Set the form to **Active**.\n\n3.  Click **Save**.\n\n\n---\n\n### **5\\. Customer Purchase Experience**\n\n1.  Go to your **Product Page** and refresh.\n\n2.  The **Custom Form** will appear on the product page.\n\n3.  Customer can fill out the form:\n\n    -   Text fields\n\n    -   Pick list options\n\n    -   Number fields, etc.\n\n4.  Add product to **Cart**.\n\n5.  In Cart, an **Information** button will display the entered form data.\n\n\n---\n\n### **6\\. View Form Responses (Backend)**\n\n1.  After **Checkout** is complete:\n\n2.  Go to **Orders** → open the relevant **Order**.\n\n3.  Select **Order Product** → check **Form Submissions**.\n\n4.  Under **Related**, view the customer’s answers.\n\n\n---\n\n## Notes\n\n-   If the customer buys **multiple quantities** of the same product, **each item** will have its **own form submission** attached to it.\n\n-   All responses are stored and viewable in the **Order Record** for easy reference."}
