# ▶️ Product form

### **1\. Create the Form**

1.  Go to **StoreConnect Navigation** → **Configuration** → **Forms** → **All Forms**.

2.  Click **New** to create a form.

3.  Choose **Product Form** type → Click **Next**.

4.  Give the form a **Name**.

5.  Click **Save**.

### **2\. Add Form Questions**

1.  Go to the **Related** tab of your form.

2.  Under **Form Questions**, click **New**.

3.  For each question:

    -   Enter the **Question Text**.

    -   Select the **Data Type**:

        -   **Boolean** (Yes/No)

        -   **Number**

        -   **Text** / **Text Area**

        -   **Date**

        -   **Pick List** (options separated by `;`)

        -   **File Upload**

    -   Optionally check **Required** to make the field mandatory.

    -   Set **Position** to control the order of the list.

4.  Click **Save and New** to add more questions as needed.

5.  Repeat until all questions are added.

### **3\. Associate the Form with the Product**

1.  Go to **Product Forms**.

2.  Click **New**.

3.  Select the **Product** (e.g. *Steve Plushy*).

4.  Click **Save**.

### **4\. Associate the Form with the Store**

1.  Still in **Product Forms**, select your **Store**.

2.  Set the form to **Active**.

3.  Click **Save**.

### **5\. Customer Purchase Experience**

1.  Go to your **Product Page** and refresh.

2.  The **Custom Form** will appear on the product page.

3.  Customer can fill out the form:

    -   Text fields

    -   Pick list options

    -   Number fields, etc.

4.  Add product to **Cart**.

5.  In Cart, an **Information** button will display the entered form data.

### **6\. View Form Responses (Backend)**

1.  After **Checkout** is complete:

2.  Go to **Orders** → open the relevant **Order**.

3.  Select **Order Product** → check **Form Submissions**.

4.  Under **Related**, view the customer’s answers.

## Notes

-   If the customer buys **multiple quantities** of the same product, **each item** will have its **own form submission** attached to it.

-   All responses are stored and viewable in the **Order Record** for easy reference.
